Finding the right care for someone you love shouldn't feel this hard.

My 98 year old Dad is new to this facility. I felt so supported by Angela Wheeler in admissions. She's never been too busy to answer my questions. I had an exceptional experience with Lisa the second shift CNA. She was so cheerful and positive while tending to a sensitive bathroom situation. It's nice to know they're not just doing their job but they are truly caring for people. I find all the staff to be very pleasant and accommodating.

With my line of work, I'm in and out of many skilled rehab facilities in the area. Five Oaks has an amazing rehab department and clean and bright facilities. Lisa and Jackie are a bright light and truly care about the residents. They are always willing to welcome the residents and families into their office and talk or pray with them. It's refreshing to see this when this is usually such a difficult and stressful time for the resident and family. Having a staff that truly cares can make all the difference and I've seen this first hand at Five Oaks. I would recommend this facility to anyone needing skilled nursing care in the area.

All I can say is, Sara and Angela have made my mom's transition into short term rehab as seamless and worry-free as possible!! When you're already going through a stressful time, it is important to be able to have a sound mind when your family member is going through so much. Thank you, Five Oaks, Sara, and Angela for making this process as easy as possible for us and for making sure my mom has felt comfortable & at ease throughout this whole process!

Our 96 year old Dad was admitted to 5 Oaks Care Center after being hospitalized for a heart condition. It was determined that he was in the end stages of his life and would need nursing home care until the end. We thought first of 5 Oaks because our mother transitioned at 5 Oaks five years ago and we were most impressed with the quality of care that was provided to her. This level of care and more has been provided to our Dad since he arrived. Angela, the admissions director was extremely helpful in facilitating my Dad's admission. Jackie, the social worker, helped our family to enable our Dad's stay to be as comfortable as possible. We met the Doctor, the Nurse Practitioner and the clinical team who supervise our Dad's care and ensured that he received the comfort medications and nourishment that he needs. CNA McKinsey and the entire staff at 5 Oaks have been so attentive to our Dad. Every request that we have made has been responded to with the utmost level of professionalism, dignity and respect. No one would wish that their family member would need this level of care. But, if they did need it, we would highly recommend that they consider 5 Oaks first, and they won't be sorry.

My mother was recently at the facility and I was beyond pleased. I was hesitant to send her there after reading some of the reviews, but I can honestly say my mother received the best care and I always felt good leaving her in the evening. The staff were friendly, the food always looked delicious and I loved seeing the fun activities my mom attended (and funny antics of the staff participating in the activities). The speech therapist Sarah was amazing, The Dietician Stephanie worked closely with my moms food preferences and always communicated with me when there was an issue and Dixie was available day or night. the staff really did care . thank you Five oaks.

My brother had some medical issues and need rehabilitation as part of his healing journey. I contacted the center from the person who answered the phone to Ms. Angela who worked with us to get all the paperwork done, it was awesome. When he got there he received excellent care from the entire staff. The nurses and all who assisted with his care provided timely updates about his care. We were often greeted with friendly smiles. The team asked questions and provided a plan and opportunity for us to ask questions and understand his care journey. Thank you ALL.

My grandfather received truly exceptional care at Five Oaks in Concord. Our family could not be more grateful for the compassion and dedication of the staff. From the moment he arrived, the team ensured his comfort and provided a welcoming, supportive environment. The nursing staff goes above and beyond attending to his medical needs and professionalism while also taking the time to engage with him personally. The housekeeping and dining staff also made his stay comfortable. His room is always clean and the meals are tailored to his requirements. The exceptional care he is getting shows how Five Oaks holds their commitment to excellence and compassion. Thank you everyone, a grateful grandson.

They are excellent. The staff is qualified and personal. It has recently been refurbished and it is so nice. Everyone there is helpful and so nice. They took great care of my Mother Pat Kelley. If the need for this care is needed again, this would be my first choice.

I am so grateful to 5 Oaks. I came here to recover from surgery. The people who work here are so kind. Wanda is the receptionist. She is the first person I met, and she made me feel at home. Marie is one of the activities directors. My favorite thing is when she paints our nails. She is patient and gentle. Johnathon is our driver. He gets us to our Doctor's appointments. He is cheerful and funny.
